i applied after 5 months of expiry. 

 

you have to apply in NY consulate... it comes back to you in 1 month.

follow the procedure in below link

$91.20

 

for shipping ok $50 esko..

for documents..inko $50 esko...if u dont hav scanner copier etc for makin copies
