Unique style: Rajnikanth has an unique style of dialogue delivery and stunt sequences. His ability to do comedy, emotion, dance and fight with equal ease attracts people of all generations. He is sort of a Robin Williams + Silvester Stallone + Dustin Hoffman combination (although the combination sometimes ends up close to Chuck Norris). 

David vs. Goliath: He made his career in blockbuster movies where he acts as the David against the Goliath of social evils. For people who are angry about their government and the various social evils surrounding them, his movies are an instant hit.

Consistency over a very long period: Rajnikanth has acted for 40 years without much slack and provided a consistent performance throughout that time. Especially after the troublesome first 5 years where he ran into trouble with the preceding superstar, he has been known for consistency & self-control. He was a superstar years before I was born and he is a superstar even now.  Most popular actors get derailed by their own popularity. Rajni kept his cool and stable head.

Likability: He has a very likable on-screen and off-screen personality. Unlike most popular actors he avoids the attitude and avoids doing anything silly. His emphasis on spirituality is especially a plus in that part of India.

Formulaic Movies He picks up highly populist movies with a strong emphasis on a specific formula (traditional fight of good vs. evil, rags to riches story where he becomes successful in 5 minutes, a good father/husband/son, 5 popular songs per movie, a couple of entertaining action sequences and a couple of comic parts). This makes for good entertainment. People of that state are very susceptible to formula. 

Spacing out the hits: He spaces out his movies well. In his first 10-15 years, he acted a lot of movies of very diverse roles. However, in the past 25 years, he acted 1 movie per year [most Indian actors do 5-6]. The movies will be released at the right time [just before a holiday]. This allows people to not get too satiated with the formula and have enough for the holidays. People could tolerate one formula movie a year and usually it will be Rajni's movie.
