Subject or record for this request: Yes(Click the box) Full Name at the time of entry to US 4a. Last Name 4b. First name Other information 5. I-94 Part 4. Mailing address 2b, 2c, 2d, 2e, 2f Other information 3. Date of Birth: 4. Country of Birth: Contact Information 6. ur Mobile: 7. ur e-mail: Requestor's certification 7a. Sign in front of notary better 7b. Date (This can be printed before taking print out) Part 3. 1.Purpose.: Need I-140 Approval notice along with labor docs. Full name 2a. Last name 2b. First Name Parents Names 13a. Father L Name, 13b. Father F Name 14a. Mother L Name, 14b. Mother F Name 15. Need I-140 Approval notice along with labor docs. Part 4 1a. L name, 1b. F Name 8a.
