Procedure to get it , posted in Quora given in a detail way.

Here is the procedure:

If your birth is registered in any records (i.e, either in Hospital or other):

1. Take a print out of this form : http://tg.meeseva.gov.in/DeptPor...
2. Fill the form and approach your nearby MeeSeva centre.
3. Hand him/her the filled form. He/She will scan the form and apply it online.
4. Pay him/her the amount and he/she will hand you a receipt with Transaction and Application ID.
5. If your application gets accepted, you will receive an SMS stating that your application has been accepted.
6. In case your birth is not registered any where in records, you will receive an SMS stating the reason for rejecting your application.

If your birth is not registered in records any where:

1. Take a print out of this form : http://tg.meeseva.gov.in/DeptPor...
2. Fill the form and attach the relevant documents as detailed in the form.
3. Approach the nearby MeeSeva centre.
4. He/She will scan your application as well as documents you attached and apply it online.
5. Pay him/her the amount and he/she will hand you a receipt with Transaction and Application ID.
6. Now with a pen write the transaction id on top of the form clearly and attach other documents.
7. Take these documents to your corresponding Mandal Revenue Office and submit it there.
8. If your application gets accepted, you will receive an SMS stating that your application has been accepted.
