Any  Immigrant Visa applicant is subject to "Grounds of Inadmissibility" rules. They will check to make sure you are not inadmissible based on those list of rules. Medical related requirements are part of Grounds of Inadmissibility. It doesn't really make sense for EBs(who have been staying in the US for a while) but it does for FB(Family based) immigrants or others who are just coming in.
