H1 and H4 last stamped in June 2017 in India. When I was with Employer A.

Now, I moved to a new Employer B. 

Planning to go to India in October.
 

As per the site, these are the questions 

Do you have previous US visa in any visa class issued ? Yes
Was your prior visa annotated “Clearance Received” or “Department Authorization” ? No
Was your most recent U.S. visa lost or stolen or cancelled? No
Have you ever been refused a U.S. visa before? No
Was your most recent U.S. visa issued after January 1, 2008? Yes
