H1B Extension - Contractor and Remote Job

Hello DB members , need some suggestion.

h1b extension vasthundi , Nenu contractor - Client location Maryland , Nenu Texas lo unta due to remote. 
ippudu extension apply chesey appudu , primary location client address ivvala or my resident address in Texas ? Can we add secondary location as well.

If we go to visa stamping in future , client location and address adigithey - LCA lo na primary address untay emaina problem aa ?? Just want to make sure visa officer don't feel I am on Bench with out project

How to handle this , so that I can have smooth extension and stamping
