I agree with thebramabull--> Prepare for reality.

 

Historically USA has more residency seats than medical seats. So all the vacancies are being filled by foreign graduates. Now they increased medical colleges in 2006-2008 period. All these new medical school graduates came in to picture now. And adding to that , there are lot of carribbean medical schools opened in 2006 time, and are still opening. These carribean school medical students,  they will do most of their rotations in USA. And almost all of these students are USA citizens,( who could not get MCAT scores and admissions in USA. 70 % of them are ABCD's.)

And lot of hospitals are being closed in the last 2-3 years, eventual decrease in residency spots.

Added to that most of the hospitals removed H1B visas to foreign graduates. Only they can get J1 visas. getting J1 WAIVER from indian government is a nightmare. between 2000-2010 there were 30-40 hospitals used to give prematches for residency for the fear of not getting candidates. In 2015 there is only one hospital in entire USA gave prematches, in 2016 even that is not going to happen.

With the competition so high now USMLE scoring requirement has increased tremendously. More Nigerians, pakistanians, Chinese, middle easterns, Russians, south americans are coming for very few spots left. Even for residency now they are applying with 2-3 research publications.

Having said all of this, this is not negative thinking, this is REALITY. I am not saying it is impossible, but this is not easy. Your brother has to understand the whole picture and better be prepared for this, so that nothing will be a surprise. Get high 99 % scores in all exams, get recommendation letters from USA doctors, do some research and publish 2-3 papers, apply in all 50 states, be prepared to apply again and again--> residency will be at his feet...................................................................................
