Try the above format for the Affidavit. You may need letter from local govt office the Birth was registered late after years of birth.

This is why USCIS does not accept late registered BC and would need NABC along with secondary evidences like affidavits, 10th certificate etc.

I guess USCIS is not asking the Non-Availability Birth Certificate.

This is from trackitt ,  some body had a same RFE with late registered BC.

 

I am just writing to the benefit of those who get similar RFE . My case was approved some time back this year. I submitted a letter from municipal corporation stating that my birth was not recorded in the year of my birth ( they take time to search for records so be prepared go ASAP). I also submitted affadavits from parents and relatives (four affadavits). Thats all I submitted.

You from Hyd? I think best thing to do is catch any broker and get a letter  saying  " For some reason, birth was not registered in the year of birth"  from  Municipal corp office . That should be enough along with affidavits .

 

Reliable brokers are best in this case , throw some money and get it done asap.

Municipal office ki vellamanu intlo vallani... brokers will be sitting outside.

No letter format bro..  I guess its a common scenario, so brokers might understand.
