You can request for certificate correction on website . you can go newspapers with your exact details

Or you can contact local health officer about the mistake and make him report the staff which entered the wrong information 

collector , courts tho workout kadhu 

Vaccine is freely available , there is no black market for govt vaccines nowadays, so most probably a lazy employee entered your details .

Certification correction only seems to be done if there is any name or correction in person details if needed not in case of erroneous update that vaccination is done.
